Authorization Forms

DSA and Approver Authorization Forms

DSA and Approver Authorization forms can be completed at UF IAM Forms. Once the form is submitted, a role request for UF_SEC_REQUESTOR for DSAs, or UF_SEC_APPROVER for approvers, should also be submitted via SailPoint.

The DSA Authorization form is used to designate a Department Security Administrator (DSA) for your department, or to initiate a change to DSA authority. Please note on the form if the new DSA will take on Primary DSA responsibilities. Only one Primary DSA is permitted per department ID. This form must be completed and signed by the appropriate vice president, dean, director, or department chair.

Note: All Department Security Administrators must take myTraining course IAM100. Primary DSAs must also take IAM200. Please ensure users have completed their respective courses prior to form submission.

The Approver Authorization form is used to designate security role approvers who will review and act on requests made by Department Security Administrators (DSA) to grant or remove access to administrative computer applications for people in their department, college or division. These approvers are members of UF SailPoint IdentityNow governance groups.

Note: All Role Approvers must take myTraining course IAM300. Please ensure users have completed that course prior to form submission.

Conflicting Role Definition Forms are reserved for use by senior core office and audit staff.  (Note: The majority of these requests will require the use of Form 1.)

Once you have completed the form, please submit a ticket via TeamDynamix and attach the form to it.

New Role Forms

This form is used to request the creation of new SailPoint roles. It will require determining the appropriate ISM, Data Classifications, Governance Group, P-Level, and more.
You may need to look-up the appropriate ISM. You can use this page to find your Information Security Leadership.
